If your child is currently enrolled in school, we require forms to be completed from your child's current teacher and the school's director. Parents/guardians are responsible for entering the email of the school personnel who will complete these forms. In addition parents/guardians will download, sign the Transcript Release form and submit it to their child's current school. This will give the school permission to release the records. Forms and submissions are made in the parent portal checklist. The forms are completely confidential.
Applicants who have not attended a formal school do not need to have completed forms. Forms for playgroups, extracurricular classes or drop off programs are not necessary.
Forms cannot be submitted to Stevens before October 1 of the current school year.
